Healthy Impact Grant Program (over $15,000)
In accordance with our Grant Guidelines, the Foundation seeks to fund projects which improve the health and wellness
of people living in the Mat-Su Borough through our Healthy Impact grant program.
Mat-Su Health Foundation accepts Healthy Impact grant requests in excess of $15,000 biannually in January and
June of each funding year. Please consult the MSHF grant guidelines located on our website for additional
information on what Mat-Su Health Foundation does and does not fund. The two cycle periods follow:
Cycle One:
Project Pitch - November 28 to January 6
Full Application - January 16 to February 24
Cycle Two:
Project Pitch - April 23 to June 1
Full Application - June 11 to July 27
All pitches/applications must be submitted electronically by 4:00 pm on the day of the aforementioned deadline.

Healthy Impact Grant Application Process
Mat-Su Health Foundation has transitioned to an electronic format. All applications must be submitted in this manner or they will be returned to the applicant.
Step 1 - Project Pitch
All applicants seeking funding under the Mat-Su Health Foundation's Healthy Impact Grant Program must first submit a
Project Pitch within the timeframe indicated above using our online application. Please click on the "Project Pitch" link above. The pitch must be submitted online. Project Pitches will not be accepted if provided through another mechanism.
You will need the following information to complete the Project Pitch:
- Legal Name, Organization Name (d.b.a.) and Tax ID Number
- Organization Type
- Physical Address and Mailing Address (if different from physical address)
- City, State and Zip
- Website Address
- A brief description of your organization
- Primary Contact Information
- Prefix, First Name, Middle Name (if applicable), Last Name, Suffix (if applicable) and Title
- Office Phone and Office Fax
- Email Address
- Executive Director/CEO Contact Information
- Prefix, First Name, Middle Name (if applicable), Last Name, Suffix (if applicable) andTitle

Step 2 - Confirmation & Invitation
You will receive and email confirming receipt of your Project Pitch. This email will also include a link that you can use to log back into your account to complete your application if it is accepted. Healthy Impact payment requests and reporting requirements are still submitted using the pdf form. As the online process progresses, information will be provided to our grantees online.
You can also use this link to forward copies of grants to coworkers for review, or to access a copy of all of your grant applications submitted for future reference.
Step 3 - Full Application Process
Once you receive notice that your project pitch has been accepted and you are invited to complete the full application, you will be able to access the rest of the application by logging into your account.
Please provide the following information about the organization requesting funds:
- Application: Information will be transferred from your project pitch to your application. These fields can be changed if they are incorrect. You must complete any remaining fields to complete the application.
- Attachments: A complete application must have all of the following attachments submitted:
- For your application to be considered the following completed attachments must be uploaded to this application in pdf format.
- IRS Letter: A copy of your letter from the Internal Revenue Service verifying your tax status. Acceptable Format: PDF Only
- List of Board of Directors: Please provide a list of your current Board of Directors including address. Acceptable Format: PDF
- Contributor/Donor List: Please provide a representative list of Corporations and Individuals that have donated to your organization in the last twelve months. Acceptable Format: PDF
- Two Letters of Support specific to the Proposed Project: Acceptable Format: PDF
- Most Recent Year-End Balance Sheet Available: Acceptable Format: PDF
- Income Statement (Profit and Loss) for Most Recently Completed Year: Acceptable Format: PDF
- Current Year Operating Budget: Acceptable Format: PDF
- Budget to Actual Comparison for Most Recently Completed Year: Acceptable Format: PDF
- Brief history of the organization; mission and vision; services provided
- Geographic area served
- Number of beneficiaries in Mat-Su and total
- Description of the project for which funding is requested
- Identification of the need for the project specifically as it relates to the Borough Health Factor(s) highlighted in your Project Pitch; and its current status
- Implementation timeline for the proposed project
- Description of total cost of the project, including a list of specific items and costs
- Source and amount of any project funds raised to date or pending; the amount requested from MSHF; and if relevant, how the organization anticipates raising the balance.
Before submitting your grant application, please ensure that it conforms to the following criteria:
- Application is complete and accurate. MSHF will not consider incomplete applications or applications that are submitted after 4:00 p.m. on the closing date.
- Project budget is arithmetically correct.
Step 4 - Notification
All grantees will be notified of their grant application's funding status no later than 90 days following the close of the applicable grant cycle.
Step 5 - After Submission
Once you have entered grant information, if you closed the grant prior to completion and need to finish the grant, click here. Enter your email and password, and you will have access to all grants associated with your email address. The information is divided showing incomplete applications first and submitted applications at the bottom.
