All applications must be in by 4:00 pm on the day of the deadline.
Step 1 - Project Pitch
All applicants seeking funding under the Mat-Su Health Foundation's Healthy Impact Grant Program must first submit a one-page
Project Pitch within the timeframe highlighted above. The letter may be submitted via fax or postal service and must be received
by our office by 4:00 p.m. on the last day of the Project Pitch cycle. The one-page Project Pitch should include the following
pertinent information related to your project:
- Project Title
- Statement of your non-profit organization's tax status
- Federal Employee Identification Number
- Identification of Healthy Alaskan 2010 Leading Health Indicator that project addresses (see attached)
- Brief narrative of intended program/project
- Brief description of how funds requested will be used
- Anticipated total cost of project
- Amount of MSHF request
- Complete agency information: mailing address, physical address, and e-mail
Step 2 - Confirmation & Invitation
MSHF will confirm receipt via e-mail of your Project Pitch within one week. Mat-Su Health Foundation will notify prospective grantees on acceptance into the grant queue or declination by the first day of the application period. Those accepted into the queue must complete a full application as outlined below in Step 3 to be considered for funding.
Step 3 - Full Application Process
Please provide the following information about the organization requesting funds:
A. Application Checklist: All applicants must utilize the MSHF Healthy Impact Grant Application Checklist as the coversheet for your completed application. This one-page form will help the applicant to ensure that the packet is complete before submittal. Checklist/Coversheet
B. Application Narrative: Compose a narrative no more than three (3) single-spaced pages that provides the information in the order requested below.
1. Brief history of the organization; mission and vision; services provided
2. Geographic area served
3. Number of beneficiaries in Mat-Su and total
4. Description of the project for which funding is requested
5. Identification of the need for the project specifically as it relates to the Healthy Alaskan 2010 Leading Health Indicator (provided below) highlighted in your Project Pitch; and its current status
6. Implementation timeline for the proposed project
7. Description of total cost of the project, including a list of specific items and costs
8. Source and amount of any project funds raised to date or pending; the amount requested from MSHF; and if relevant, how the organization anticipates raising the balance.
C. Attachments: A complete application must have all of the following attachments submitted:
1. Letter from the IRS verifying the organization's 501(c)(3) status
2. List of board directors with affiliations for each member
3. A representative list of contributors, grantmakers, and donation amounts to your project and organization, including the MSHF, during the preceding fiscal year. Please limit this list to a single page.
4. At least two letters of support for the proposed project from project partners, community entities that the project effects, or other applicable entities
5. Most recent balance sheet available
6. Twelve-month income statement for most recently completed year
7. Current year operating budget, showing anticipated sources of both revenues and expenses.
8. Budget to actual comparison for the most recently completed year.
All required attachments must be received by our office before the closing date of the applicable application period. Before submitting your grant application, please ensure that it conforms to the following criteria:
- Application is typed.
- Application checklist serves as your application's cover sheet and all documents appear in the order specified on the checklist.
- Applications and attachments are paper clipped. Please do not use staples, binders or elaborate packaging.
- Application is complete and accurate. MSHF will not consider incomplete applications or applications that are submitted after 4:00 p.m. on the closing date.
- Project budget is arithmetically correct.
- Application cover sheet checklist is signed by the applicants Executive Director.
Step 4 - Notification
All grantees will be notified of their grant application's funding status no later than 90 days following the close of the applicable grant cycle.
There are Two Funding Cycles Each Year
Completed grants applications must be submitted by the 4:00 p.m.
- February 26
- July 30 (excluding weekends)
Applications received during the respective funding cycles will be reviewed following the cycle deadline. Applicants will be notified once a decision has been made.
Target Wellness Grant Program
Mat-Su Health Foundation accepts Target Wellness grant requests up to $15,000 throughout the year.
Please consult the MSHF grant guidelines located on our website for additional information on what
Mat-Su Health Foundation does and does not fund. Grant requests for health and wellness projects in Mat-Su
(or hat serve Mat-Su residents) are accepted on an ongoing basis throughout the year.
Eligible applicants include 501(c)(3) not-for-profit organizations physically located within the Mat-Su Borough.
Application Process
To apply, please complete the following:
A. Application Checklist: All applicants must utilize the MSHF Target Wellness Grant Application Checklist as a coversheet for your completed application. This one-page form will help ensure your packet is complete before submittal.
B. Application Form The Target Wellness Grant Program application should be completed entirely on the form found at www.matsuhealthfoundation.org. Each section should be filled out in its entirety and without additional items except those noted below.
C. Application Attachments Please attach the following documents to the application form and checklist.
1. Two letters of support from agencies or businesses related directly to the proposed project.
2. Letter from the IRS verifying the organization's 501(c)(3) status.
Application Review and Evaluation
A. Once a completed application is received by the Mat-Su Health Foundation, staff will perform an internal review of the proposed project. Staff may phone the grantee if additional information or clarification is required related to the proposed project.
B. MSHF staff will inform the Grantee of the status of their funding request within 30 to 45 days of receipt of a completed request.
C. If awarded, the Grantee will receive a Grant Agreement to be signed by the Executive Director or designee, instructions for grant reporting, and release of project funds.
Alaskan 2010 Leading Health Indicators
1. Physical Activity Increase the proportion of adolescents (grades 9-12) who engage in vigorous physical activity.
2. Physical Activity Increase the proportion of adults who engage in regular preferably daily moderate physical activity.
3. Overweight & Obesity Reduce the proportion of adolescents who are overweight (grades 9-12).
4. Overweight & Obesity Reduce the proportion of adults who are obese.
5. Tobacco Use Reduce cigarette smoking by adolescents (grades 9-12).
6. Tobacco Use Reduce cigarette smoking by adults.
7. Substance Abuse Increase the proportion of adolescents not using alcohol or illicit drugs during the past 30 days (grades 9-12).
8. Substance Abuse Reduce binge drinking among adults.
9. Mental Health Reduce the suicide rate.
10. Injury Prevention Reduce deaths caused by unintentional injury.
11. Injury Prevention Reduce deaths caused by motor vehicle crashes.
12. Violence Prevention Reduce deaths from homicide.
13. Violence Prevention Reduce child maltreatment.
14. Immunization Increase the proportion of young children who have received all vaccines recommended for universal administration.
15. Immunization Increase the proportion of elderly adults immunized against influenza and pneumococcal disease.
16. Environmental Quality Increase communities access to safe water and proper sewage disposal.
17. Environmental Quality Reduce the proportion of nonsmokers exposed to environmental tobacco use.
18. Access to Health Care Decrease the percent of Alaskans without health insurance coverage throughout the year.
19. Access to Health Care Increase the proportion of adults with a usual place to go for care if sick or needing advice about health.
20. Maternal and Child Health Increase the proportion of pregnant women receiving adequate prenatal care.
21. Maternal and Child Health Reduce post neonatal death rates.
22. Responsible Sexual Behavior Increase the proportion of adolescents who abstain from sexual intercourse (grades 9-12).
23. Responsible Sexual Behavior Increase the proportion of adolescents who use condoms (grades 9-12).
24. Responsible Sexual Behavior Increase the proportion of sexually active persons who report condom use at last intercourse.
